Disease Management

The Disease Management program uses evidence-based practice standards and current clinical practice guidelines to assist you with your chronic disease process and self-management goals.

We aim to empower and improve your health and well-being by providing health coaching and education to assist you in reaching your wellness goals.

Our program collaborates with you, your PCM, and the rest of your health care team to develop a joint plan of care to improve your understanding of your disease process and promote/maintain optimal health and wellness.

The Disease Management Program is open to all beneficiaries.

Who are Disease Managers?

Disease managers are highly trained registered nurses who work in a specialty referral clinic.  We receive referrals from Primary and Specialty Care providers.  You or your family member may also self-refer.

Why Would I Need Disease Management?

Disease Management has the unique opportunity to provide you with a longer appointment time than normal clinic appointments, which allow you more of an opportunity to discuss your specific healthcare concerns and needs.

Disease Managers seek opportunities to empower you to better understand your disease process, and prevention or minimizing long-term complications.  We can also discuss lifestyle interventions and medication management.

Disease Managers can assist with this by providing one-on-one appointments (in person or over the phone) and group classes that use a combination of education and motivation to reinforce self-management.

What Disease Management Services are Available?

We can assist you with the following conditions:

  • Asthma
  • Chronic Obstructive Pulmonary Disease (COPD)
  • Congestive Heart Failure
  • Diabetes
  • Hypertension (High Blood Pressure)
  • Hyperlipidemia (High Cholestenol)
We also offer Diabetes and Heart Healthy Class once a month.

Heart Healthy Class

Would you like to learn more about high blood pressure, high cholesterol, heart failure, or overall heart health and treatment options? Join this class and learn more about your diagnosis and healthy lifestyle changes including a heart healthy diet.
Speakers:  Cardiologist, Nephrologist, Nutritional Medicine, and Disease Management

Diabetes Class
