Non-fly personnel PHA/MHA

PHAQ: PLEASE NOTE WE NOW COMPLETE THE PHA & MHA TOGETHER!
Step 1: PHAQ -- go to ASIMS, click on the "Start PHAQ" button and complete the questionnaire.
Step 2: PHA/MHA. After completion of PHAQ, the next step is the Mental Health Assessment (MHA). PHA/MHAs are over the phone interviews with a credentialed provider. To schedule an appointment, please call 702-653-CARE (2273). Please have your DoD ID number readily available. When calling, select the following options:

• Select option 1 to make an appointment
• Select option 1 if you're Active Duty
• Select option 2 for Non-Flyer assigned to Nellis (Despite being assigned to Creech, please select this option)
• Select option 1 to schedule your MHA/PHA

Step 3: Record Review -- no action required from member, conducted by Base Operational Medicine Clinic (BOMC)
Step 4: Provider Review -- no action required from member, conducted by Base Operational Medicine Clinic (BOMC)

Flying Personnel

If you are a Flyer, your PHA and MHA will be conducted at Flight Medicine during your annual Flight Physical. Once PHAQ is complete, if stationed at Nellis contact Flight Medicine or stationed at Creech contact the Hunters Medical Clinic to schedule your annual flight physical.

If you wear glasses or contacts please wear your military issued glasses the day of your appointment (No contacts allowed).

You are due for a DrHA/DHA if you have recently been tasked with a new deployment or have returned from a deployment. DrHA is the online questionnaire, and the DHA is the appointment with a provider.  Once you complete the DrHA questionnaire, please call 702-653-3439 to schedule the DHA Appointment. DHA 1 & 2 are in-person appointments, and DHA 3, 4 & 5 are over-the-phone.

This is an introduction for the medical separation/retirement process. Please follow the steps and contacts listed as well as the information given to you through Transition Assistance Program (TAP) and via the separation-briefing meeting.  Please note: SHPE records are printed.  If you need copies contact Medical Records at 702-653-2043

- Log into your IMR via the AF Portal, click on the SHPE tab, follow the on-screen prompts and instructions to initiate the SHPE Questionnaire and MHA, and then reach out to a SHPE nurse at 702-653-2074 or 702-653-9724. If the SHPE/MHA questionnaires are not complete, we will not schedule you. If you are planning to see the VA, do not complete the SHPE MHA, and contact at a SHPE nurse at 702-653-2074 or 702-653-9724 for further instructions.

- Your SHPE (separation/retirement) process can start 179 days from your final out date. We would like to schedule you for your SHPE exam with the provider before you take terminal leave. For further guidance or questions, please contact a SHPE nurse at 702-653-2074 or 702-653-9724.

In ASIMS, select "New Medical Clearance Request", and choose the appropriate types of request being submitted.  After submitting your request, you can check your status in your IMR.  Answer the medical questionnaire within the application and then send your completed documents by encrypted email to usaf.nellis.99-mdg.mbx.99-amds-sgpf-bomc@health.mil. NOTE: If you initiated a clearance through your MyIMR, and do not complete/submit it within 72 hours, the system will delete your request and you will need to re-submit your request.  To receive status updates on your request, email usaf.nellis.99-mdg.mbx.99-amds-sgpf-bomc@health.mil.

Retraining/Re-class

• Attach your retraining myPers response email in a secure message via email to usaf.nellis.99-mdg.mbx.99-amds-sgpf-bomc@health.mil
• Once the 422A is complete (14-21 duty days), it can be downloaded from the AF Portal under MyIMR
• Flying AFSCs will take 10-12 weeks (please contact Flight Medicine for instructions)
• If you wear contact lenses, do not wear them for 30 days prior to your Flight Physical.

Overseas Medical Clearance (Active Duty only)

• PCS clearances are processed no earlier than 120 days from RNLTD. Please include your report date in the "additional comments" section.

Palace Chase/Front 422A Request

• Complete the medical questions and provide a tentative separation date.
• Submit documentation to usaf.nellis.99-mdg.mbx.99-amds-sgpf-bomc@health.mil
